How Do I Figure Out What's Clobbering My Sound?

At some random time something clobbers my sound. Tones change to
beeps and other sound anomalies begin. When it happens I have to
reboot to fix it. Sometimes I can go for many hours before this
happen, other times it happens soon after a reboot.

It seems clear that something that runs occasionally is doing it but I
    can't figure out what.

How do I determine what's causing this?

Since there may be no reason for sound for several minutes to an hour
or so of operation there's no way I can remember what programs I've
been running. Is there a utility that lists all the apps that have
    run in chronological order?
