Consider the cost of production for a single sheet folded insert. Most
require extensive graphic design, typography and layout involved in their
creation. If you notice, most are designed like thematic movie posters. That
is the real cost of production. You need a second team alongside the DVD
production team to do the inserts. Their work has to be coordinated. There
is more involved that printing a sheet of paper. The production of a
mult-page booklet with typography and layout like Criterion inserts is even
more involved. The expertise required of a typographer to choose just the
right font for the theme and lay the text out is non-trivial for that
    trivial insert.
