inexpensive 800 origination

Awhile ago someone posted about a service that looked almost as good
as LiveVoIP (which I was using then) as far as 800 origination went.
I've been unsuccessful at locating that message. LiveVoIP just
tanked and I'm looking for another service where I can get toll-free
numbers for ~$1-2/month and $0.01-0.03/minute with >=4 simultaneous
    calls/line.

I just tried Teliax and I've been pleased with their responsiveness
but their rate plans don't quite fit my needs. Junction Networks has
    $2/month toll-free numbers but they charge $0.039/minute.

I'm making my way through the list at voip-info.org but I'll
appreciate advice on good VoIP providers who offer the service I seek.

    Thank you.
