Excel To Word - How To Format?

    I want to create a Word file as follows:

    Smith, John
    XYZ Co.
    Fax#: 724-123-4567

    Brown, Mary
    ABC Co.
    Fax#: 724-321-4567

    Powers, Austin
    Loco Co.
    Fax:# 724-541-8000

    I have an Excel file with the data, but I don't know how to set the format
    up? Any help?

    In Word, go to Tools - Mailmerge and follow prompts.
