My question is with type of release of IOS (WC) is it advisable to
place in a production environment, and can it be attributable to
switch reboots? What other things could we check for, pretty general,
but just wanted to bouce it off the group?

My guess is that 12.0(5) is pretty ancient and that something
    more recent will be available.

if you want to post the output from:-
    sh stacks
    sh ver
sh tech - perhaps edited to disguise sensitive addresses

Then perhaps someone might be able to asist.

Check for crashdump files on your flash too. I forget
the exact names but it is pretty obvious. If you have
any let us know and perhaps you could mount the
most recent somewhere that someone could have
a look. These may contain sensitive data that you may
well not want to release. If it is an internal switch then
you should be reasonably safe with regard to sensitive
    material.

    show flash

You should consider enabling buffered and syslog
logging. You might then be able to see early
symptoms of the failure. for example memory
    exhaustion.
