pesky file - removal help

Have a customer who has the file : thnall1m.exe. From what Ive read its a
trojan that monitors itself, and replicates if the file has been removed. It
hasnt been installed, as his McAfee firewall catches it. HOWEVER, the file
pops up when switching from his daughter's account to his. This is on an XP
home machine. Macafee wont remove it, Symantec didnt, online scan from Trend
didnt catch it. How can this be eradicated once and for all?

boot up in safe mode or delete the file in recovery console.
