How do you completely remove evidence of any web sites visited?

1 - reinstall your computer
2 - break into your ISP's server farm and trash all their hard drives
3 - break into their data store and burn all their backup tapes
4 - go visit the web host and perform steps 2 and 3 there
5 - trace back through every router and proxy between you and the web
site and perform steps 2 and 3

Now comes the hard bit, if the site was abroad, you might have to break
into CIA (or substitute any TLA of your choice) and do same there. This
however might draw unwanted attention to your activities.

You forgot using Symantec PC Anywhere an breaking into the FBI's Carnivore appliance located
at the ISP and wiping their hard disk
